Harborview Medical Center – Social Work has an outstanding opportunity for a SOCIAL WORKER - OB NAVIGATOR to join the OBGYN Clinic .
WORK SCHEDULE
• 40 hours per week
• Day Shift
This position is expected to last one year.
POSITION HIGHLIGHTS
The HMC OB/GYN Clinic provides team based obstetric and gynecologic care to women within HMC’s mission population in Seattle/King County, and for women from the WWAMI region with severe acute obstetric or gynecologic trauma. The Social Worker/OB Navigator will be part of teams in both the OB/GYN Clinic and the HMC Social Work Department. The Social Worker/OB Navigator will work from a strengths-based, trauma-informed and harm reduction perspective as well as practice cultural humility and use an anti-racism lens.
DEPARTMENT DESCRIPTION
The HMC OB/GYN Clinic provides full spectrum team based obstetric and gynecologic care to women within HMC’s mission population in Seattle/King County, and for women from the WWAMI region with severe acute obstetric or gynecologic trauma. The Social Worker/OB Navigator will be part of teams in both the OB/GYN Clinic and the HMC Social Work Department. The Social Worker/OB Navigator will work from a strengths-based, trauma-informed and harm reduction perspective as well as practice cultural humility and use an anti-racism lens.
PRIMARY RESPONSIBILITIES:
• Engage with patients to encourage early and ongoing use of prenatal and newborn care, including at visits with patients three times during the perinatal period: Intake/assessment, 3rd Trimester, and Postpartum
• Coordinate needed medical and supportive services consultations during prenatal and postpartum care at HMC’s OB/GYN Clinic. This includes the multidisciplinary team, as appropriate.
• Facilitate connection to perinatal-specific resources as well as community resources to address social determinants of health, toward a goal of improving psychosocial stability during the perinatal period. Resources include, but are not limited to:
o Medical coverage
o Food related, including WIC (Women Infants Children) & SNAP (Supplemental Nutrition Assistance Program)
o Transportation (Medicaid funded & community based)
o Perinatal education services & Birth Doulas
o Early Parent support, such as PEPS and Early Learning
o Pediatric & Primary Care
• Foster existing and new referral relationships with community providers serving underserved pregnant women within HMC’s mission population.
• Liaise between HMC OB/GYN Clinic and UWMC Montlake to improve patient connection to Labor and Delivery Unit (L&D). This includes a focus on facilitating culturally appropriate guided tours of UW Montlake L&D and educating patients and families about the admission process.
REQUIREMENTS:
A Master of Social Work Degree from a program accredited by the Council on Social Work Education
AND
One year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.
LEGAL REQUIREMENTS:
Within 90 days of hire must hold one of the following credentials issued by the state of Washington:
• Agency Affiliated Counselor
• Licensed Social Work Associate -Advanced
• Licensed Social Work Associate - Independent Clinical
• Licensed Advance Social Worker
• Licensed Independent Clinical Social Worker
